The preliminary investigation report suggests that the pilot monitoring (PM) confused two critical levers in the flap lever and condition lever, leading to the wrong levers being pulled and executed. This confusion resulted in a loss of power from the engines. The aircraft's stick shaker was activated at 311 feet AGL, indicating that the aircraft was close to stalling.
